Temozolamide, ondansetron이란 무슨 약일까?

Temozolamide

 

얘는 항암제, Alkylating agent으로서, 새로 진단된 glioblastoma multiforme(RT하면서 CTx 같이 할때, CRRT)에 사용되는 약제이고, refractory anaplastic astrocytoma에도 쓰인다.

상품명은 Temodar이다.

부작용으로는 Nausea, vomiting이 제일 많고, fatigue, heasdache, seizure, hemiparesis, lymphopenia, thrombocytopenia, neutropenia,를 일으킬 수 있다.

Contraindication으로 hypersensitivity to temozolomide or dacarbazine 에서 쓰이면 안됨(둘다 MTIC로 대사됨)

 

 

 

 

Ondansetron

 

Antiemetics 제재로 Selective serotonin receptor antagonist이다.

언제 쓰냐면, 케모를 한 환자에서 (특히 moderate to highly emertogenic한 케모 치료)에서 Nausea와 vomiting을 예방하기 위해 쓴다.

상품명은 Zofran이다. 잔탁이나 조신이랑 헷갈리지 말란다.

 

내가 본 환자분은 recent memory loss를 주소로 온 여환이었는데, Brain MRI상 tumor 발견되어서 수술을 하고 CCRT 위해 오신 분으로, 이틀 전부터 temodal로 chemo하시던 분으로 temodal 투여 후 5시간 정도 뒤에 nausea, vomiting, dizziness 호소 하여서 macperan 주었으나 호전 안됨.

zofran 주니 N/V 발생하지 않아서 RT동안 Zofran 유지하기로 함.

 

부작용으로는 10% 이상에서 headache, malaise/fatigue, constipation 나타날 수 있다고 한다. 확인하러 가봐야 함.

추가적으로 pruritus, rash, fever, drowsiness, dizziness, anxiety, diarrhea, 간수치 증가 등이 있을 수 있다고 한다.
